Edits to Fee Schedules do not function correctly

When editing fee schedules, the following issues occur:

1. When editing the Fee Schedule details, the Name shows an * (Required) but you can enter a blank value.

2. When editing a line item, the line details show no information for the line. If you enter details and click Save, a new line is created.

3. When editing/add a line item, and clicking save, all fields show the user must enter a value, but only the sort order field is marked as required. The window should load values of the selected Line item. When clicking save, the selected Line Item should be updated.

These issues are resolved in FIT release 1.9.

The work around until FIT 1.9 is released is to delete the old line item for issue #3 above.
