Fee Schedule Edits do not function correctly

When making edits to fee schedules, the following issues occur:

• When you edit the Fee Schedule details, the Name shows an * (Required) but you can enter a blank value.
• When you edit a line item, the line details modal shows no information for the line, does not load values of selected line item. If you enter details and click Save, a new line is created rather than updating the selected line item
• When you edit/add a line item, if you click save, all fields show the user must enter a value, but only the sort order field is marked as required.
• If you attempt to edit name on a fee schedule and click save, nothing happens
