Cannot Submit Grant Recommendations for New Grantees in DonorCentral

When a donor/advisor or foundation member viewing as advisor tries to make a grant recommendation for a new grantee, the process never completes. The advisor fills in the required information but after clicking Submit at the final step, nothing happens and the recommendation isn't made.
After clicking the Submit button, it's possible to see a green dot spin continuously.
Answer:
This issue was resolved for the customer who reported the issue and was caused by fields being incorrectly mapped in the recommendation form. In this case, CharityAddress7 was used instead of CharityAddress1 and CharityCity3 was used instead of CharityCity and lastly CharityState3 was used instead of CharityState.
Steps To Duplicate
1. Log into DonorCentral as a donor/advisor or foundation member who then clicks the View as Advisor button and selects a donor/advisor.
2. Selects Recommendation > New from top of the DonorCentral window.
3. Fills in required fields on the Charity tab, selects the Add New Charity checkbox and clicks the Next button.
4. Fills in the required information on the Recommendation tab and clicks the Next button.
5. Checks the I Agree checkbox on the Agreement tab and clicks the Review button.
6. Confirms the information on the Review window and clicks the Submit button.
7. Nothing happens (or a green dot spins continuously) and the grant recommendation isn't completed.
