When recommending a suggestion in DonorCentral, the following errors occur:
'Application exception Get form display settings failed'
'MIssing form'
'Application exception Get form definition failed'

This issue was caused by the user that was logged into DonorCentral having a staff code profile and a fund association record on a fund using the same email address however with one of the emails on one of the records having an incorrect format, or spaces which would cause two emails to be uploaded to DonorCentral which were the same email except for the formatting which caused the errors.
This issue was addressed in FIMS 14.64.
Before 14.64 there was the possibility that an email could be entered with leading spaces and email validation would not have picked it up. The email addresses would be different as far as DonorCentral uploads are concerned, thus uploading two records which causes the error.
Even though the issue was fixed in 14.64 as far as email validation, you will need to run the System Integrity report periodically to check for email issues. This issue can occur after 14.64 if the system integrity report is not run to fix incorrect or duplicate email addresses for emails that were in the system incorrectly before 14.64 was installed.
Tools > System Utilities > System Reports > System Integrity Report.
Check the box for Check DC email validation. This report checks for the following:

Steps To Duplicate
- Log into DCNXT
- Choose advisor view for the advisor for the fund in which you would like to recommend a suggestion
- Go to home tab and click Recommendations\New
- See errors occur
