    This error may occur after adding a new item to the custom menu through Dynamic Menu Maintenance and then subsequently appear for all users when logging into FIMS.

    Steps To Duplicate:
    1. Log into FIMS
    2. Go to Tools > System Utilities > Admin Utilities > Dynamic Menu Maintenance
    3. Copy the custom parent
    4. Change order to next number
    5. Type in the ODBC path
    6. Change sub menu from parent to custom
    7. Close window
    8. Prompted to reset menu bar - click yes
    9. Receive error
    10. FIMS shuts down
    11. Attempt to log in
    12. Receive error
    13. No one can launch FIMS
    Answer:
    This issue was addressed for the customer by a back-end fix done by development. This issue can be avoided by clicking Copy to create a new menu item which gives it a new order number, then do not edit anything else. Also naming a report with 'report' in the name or 'file maintenance' can cause problems.
